Here you will find forms that are used for various purposes in Job's Daughters. In regards to the new Media Release Forms. Please have one form signed for each person who will be appearing in photos submitted to the website for posting. These forms will be kept by your Bethel. After the forms are signed, then use the Signed Media Release List to let us know who has signed Media Release Forms in your Bethel. You may use the Signed Media Release List as many times as you need to get all of those who signed Media Releases reported. You may also use the Online version of the Signed Media Release List if you wish. Please remember that you must have in your possession as Signed Media Release Form before you can report that person on the Signed Media Release List.
PLEASE NOTE: Due to a change in our servers, our old Online Signed Media Release List became inconsistent. Therefore we have changed to a pdf online submission form. Simply enter the information required, click on the submit button, and it will automatically attach itself to an e-mail that you will then send to us (the address will already be filled in).
Depending on what version of Adobe Reader or Acrobat you have and how your computer is set up, you may have some problems with this process. If you are unable to send your form in this manner, please use the mail in form.

Pocket Petition Several jurisdictions now us this smaller sized petition that will easily fit in a pocket. Since it has all the information a regular petition has, it is considered to be a legal. It is suggested that it be attached to a regular petition when it is received. Print it front and back, and you will get two petitions out of each piece of paper. |
